Do you know of a child or adult who could benefit from braces? Are they a deserving individual that has done selfless things to help others in the community, their family or their friends? Wilkinson Orthodontics and CatFM radio would like to hear from you!

Nominate that person today to be entered to become a finalist in the Changing Smiles Changing Lives Contest. One adult and one child will be chosen to receive free orthodontic treatment from Dr. Wilkinson.

On the application form include:

• Your (the nominator) name and contact information.
• The name, age, and contact information of the person being nominated.
• In 500 words or less describe why you think the person who you are nominating deserves free braces. Anything over 500 words will not be considered in picking the finalists.

Other important rules:

• Last date for entries is Monday February 20, 2012.
• The essay that is submitted needs to be compelling, interesting and relevant to the contest.
• Nominations should be based on someone who you think is in need of braces and is deserving. We want to give back to someone in our community who has depicted selflessness, gratitude, an appreciation for others and is an all around genuine person who would be grateful for and benefit from this gift.
• Nominations need to be someone other than yourself.
• Age groups are 8-17(child) and 18-75(adult).
• A winner will be chosen from each age group.
